Insurance Plans
Phoenix Oral & Facial Surgery accepts most major private dental insurance PPO plans and can assist you in determining what portion of your treatment is covered by insurance. Our goal is to ensure that you receive the maximum allowable benefits given by your insurance plan. Please bring your insurance card to your first visit and we can help file your insurance benefits for you. NOTICE, we do not bill any medical/healthcare insurance. As a courtesy, we will gladly help you submit your claim to your medical insurance carrier.
Please understand, however, that you are ultimately responsible for the payment of all services. Should your insurance plan not cover what is expected, you will be responsible to pay any uncovered charges.
Payment Options
We offer a variety of financial options to ensure payment of any fees not covered by insurance will be affordable. We accept most major credit cards (MasterCard, Visa, AmEx & Discover) and also offer financing through Care Credit and Lending Club. We will be glad to discuss these with you upon your arrival.
